All kits will also include enrollment forms for plan participants to complete. As explained in Section IV (Ohio National Products and Services), the enrollment form used with the Advantage Series contracts is the Advantage Series Group Variable Annuities Enrollment and Change Request (Form 3764).
Participants who wish to select their portfolio options must complete the applicable Advantage Series Group Annuities Investment Elections Form (Form 3759-ST or 3759-F) depending on the contract type. As explained on each form, plan participants may allocate contributions among a maximum of ten portfolio options. These forms must be completed at the time the employee enrolls in the plan and again any time the participant wishes to reallocate portfolio choices. In addition, plan participants must fill out an Application for Participation/Designation of Beneficiary (Form 3732).
