The Estate Planning Data Taker (Form 2491) is designed to help you collect and assemble information that is essential in creating an estate analysis and designing an estate plan.

On Thursday, John and Mary Stevens are meeting with you in your office. After sharing the process you use to help people plan for efficient estate distribution and minimize taxes, you explain that you would like to analyze their current life insurance situation from the perspective of their overall estate plan. For that, you further explain, you would like to review the legal and financial documents requested earlier and complete an Estate Planning Data Taker. The couple agrees, and you begin to fill out the form.

The following screens illustrate some of the key data learned about John and Mary Stevens.
