Full Service Cases

With full service administration cases, Ohio National furnishes the employer with information and prepares Department of Labor reports including:

• Form 5500;
• Schedule A (Insurance Information);
• Schedule D (DFE/Participating Plan Information);
• Schedule P (Annual Return of Fiduciary of Employee Benefit Trust);
• Schedule R (Retirement Plan Information);
• Schedule SSA (Separated Vested Participants with Deferred Vested Benefits);
• Schedule T (Qualified Pension Plan Coverage Information);
• IRS Form 5330 (Return of Excise Taxes Related to Employee Benefit Plans); and
• Summary Annual Report.

Ohio National provides IRS Form 1099-R to both full-service administrative service clients and investment only clients. These are mailed directly to the participant and reported to the IRS.
