Before going out to meet with them, you'll need to order enrollment kits either through your Retirement Plan Wholesaler or through the Retirement Plan Services Sales and Marketing Department. Each enrollment kit contains educational material and all the necessary paperwork that each eligible employee needs to complete in order to enroll in the plan (which, in this case, is six of the law firm's seven employees).
Be mindful that there are different enrollment kits for each Advantage Series contract, so the paperwork inside the enrollment kit will depend on the contract you choose as the funding vehicle. For Jacobs and Lofgren's retirement plan, we are using the ONCore Advantage contract so the following forms would be necessary from all participants for their enrollment:
- Group Retirement Plans Enrollment and Change Request (Form 3764)
- Group Retirement Plans Investment Elections (Form 3765, or Form 3758 if using Stable Value)
- Group Retirement Plans Designation of Beneficiary (Form 3732)
