Full Service Cases

With full service administration cases, when applicable, Ohio National will furnish the employer with information and complete the appropriate Treasury and Department of Labor reports (refer to the Retirement Plan Services Application and Agreement (Form 3251-B) for more details). Remember, the employer is still responsible for reviewing and electronically filing these forms with the appropriate government agency. Forms that may be completed by Ohio National are:

• Form 5500, 5500-SF, or 5500-EZ Annual Return (as appropriate)
• Schedule A, Insurance Information
• Schedule C, Service Provider Information (if applicable)
• Schedule D, DFE/Participating Plan Information
• Schedule G, Financial Transaction Schedules (if applicable)
• Schedule H or I, Financial Information (as appropriate)
• Schedule R, Retirement Plan Information (if applicable)
• Form 8955-SSA, Separated Vested Participants with Deferred Vested Benefits (if applicable)
• Form 5330, Return of Excise Taxes Related to Employee Benefit Plans (if applicable)
• Form 5558, Application for Extension of Time to File Certain Employee Plan Returns (if applicable)

Ohio National provides IRS Form 1099-R to both full-service administrative service clients and investment only clients. These are mailed directly to the participant and reported to the IRS.

Ohio National does not provide actuary or plan auditing services for government reporting.
