In addition to the items mentioned, plan installation involves paperwork associated with Ohio National's products and plan services. Advantage Series group variable annuity contracts are simple to install. For any Advantage Series contract, the plan sponsor submits the appropriate contract application (and attachments if necessary) and participant enrollment/investment election forms to Ohio National. The Retirement Plan Services Department takes over from there.
Each Advantage Series contract has separate forms required for installation and can be easily obtained on the ONFS Forms Catalog. The required forms for each contract are outlined on the next page, based on Investment Only Services. For expanded retirement plan services such as full service administration, additional forms would apply. Please note, Stable Value Fund Enrollment and Change forms are also available for ONCore and Portfolio Advantage contracts.
