ONCore Advantage
- Submit the Request for Proposed ONCore Advantage Application (Form 6325 or specific state variation) and the ONCore Advantage Worksheet (Form 6326).
- Also submit a Retirement Plan Services Advantage Series Enrollment & Change Request (Form 3764) and Investment Elections Form (Form 3759, 3759-ST or 3759-F) for each participant.
- ONESCO Registered Reps must also complete Form ONESCO-1 to be submitted with each application.
